Insurance:
Each patient is either a "self-pay" (no insurance) or insured. Self-pay patients are subject to paying at time of service in accordance with our published fees. Insured patients that are insured with companies with whom we are contracted are required, prior to their first visit, to obtain an authorization number from their insurance company for mental health treatment (called and "Initial Visit"). We will NOT schedule an appointment without your providing the authorization number given to you by your insurance company. Upon arrival at our facility for the first time, all new patients will be asked to complete our intake forms. PLEASE READ THEM CAREFULLY. If the patient is a minor, that minor's parent or guardian must complete and sign the intake form for a minor (and becomes the "responsible party"). Insured patients will be asked for evidence of their insurance, usually an insurance card. We will make a copy of the card. Insured patients, if all the above is completed, will only be responsible for paying their "co-pay" immediately after their visit. We will file a claim with the patient's insurance company for the remaining amount due. |
